Timing is critical when planning a fetal echocardiogram. Choosing the right week of pregnancy ensures the heart structures are large enough to be imaged clearly, while leaving sufficient time for clinical decision-making, counseling, and further tests if required.
The optimal window for a comprehensive fetal echo scan is between 18 and 22 weeks of pregnancy.
By 18 weeks, the fetal heart is large enough for detailed anatomical evaluation of chambers and outflow tracts. Additionally, the baby's bones are still mostly cartilage, meaning they cause minimal shadowing and allow clear passage for ultrasound waves.
Compared to earlier weeks, the baby's position is more predictable and the cardiac axis is stable, making it easier to consistently acquire all standard views.
The development of the great arteries and heart valves is complete by mid-gestation, allowing a highly accurate assessment of outflow tracts, conotruncal anomalies, and aortic arch anatomy.
Blood flow velocity across heart valves is stable enough at this stage to permit accurate Doppler studies without early-gestation artifacts.
In certain high-risk situations, an early targeted scan can be performed starting in the late first trimester (11–13 weeks) or early second trimester (14–16 weeks).
When is early scan recommended? It is offered when there is increased nuchal translucency, abnormal first-trimester screening, a strong family history of heart defects, or when early routine scans raise a suspicion.
Important Limitation: Early scans can identify many severe defects, but they cannot replace the comprehensive 18–22 week study. Because the heart is still very small, all early scans require confirmation during the 18–22 week window.
For families who receive a diagnosis of a significant heart condition, the timing of diagnostic reports is legally important under the Medical Termination of Pregnancy (MTP) Amendment Act 2021 in India:
